Entry statement form

No later than 24 hours before entry to Israel, fill out an entry statement form. You must provide your personal information and that of the people traveling with you, a health declaration, mention whether you are vaccinated or recovered, and provide your isolation address, if mandated.

Exit statement form

No later than 24 hours before leaving Israel, fill out an exit statement form. You must provide your personal information and that of the people traveling with you, and check a statement that you will not visit a red destination without permission.
